Independent Contractor Agreement

Applicants must be at least 14 years old. This agreement is between Plano Sports Authority, Inc. and the independent contractor named below. The contractor controls the manner and means of the services, supplies required equipment, is responsible for applicable taxes, and is not an employee eligible for PSA employment benefits.
